Creating a Report returning the label of a null field in a form.

Hi everyone,

I have a form that we use for data entry for our clients. It consistes of
about 20 or so date fields. I would like to create a report that will return
The name and other identifying information then only the labels on the form
of the date fields that are blank.

Any ideas how to do this?

Thanks in advance!

Maybe create a query where you put IsNull as criteria in the datefield and
base a report on that query?
