Creating an index on a publication

Hello,

I am doing a publication of raffle tickets in publisher and I want to create
an index of up to 1500. This index which should be on both copies of the
raffle ticket - i.e. the one that is being retained and the one that goes to
the buyer.
Example of one ticket should look like

Christmas Party 2004 Christmas Party 2004
Raffle Raffle

001 001


Name Name
Extension Extension


Price: Le2,000 Price: Le2,000


Can anyone help me on this urgently?
