Creating and sending an e-mail based questionnaire

Hi,

I am interested in learning how to create an e-mail based questionnaire for my college assignment. How can I create an e-mail that contains textboxes and other controls so that the recipiant can fill the questionnaire out, or possibly even fill out an e-mail based form? I am farily confident with the use of Outlook and can program in VBA script for Microsoft Office. If it is possible, would somebody be able to help me out with this problem.

Any help would be greatly appreciated!
