I have been assigned a task of creating an questionare in excel in which the
person answering the questionare has to select from given limited options. I
would like to take the help of check boxes and radio buttons for this
purpose. I would require a briefing of how to create these checkboxes and
radio buttions.

You first need to open the form toolbar from the menu

View- Toolbars - Forms

There are 4 items you need

1) Check boxes
2) Control button
3) Group box to group Control buttons so you can only select one at a
time. Place Group box down and then place the Control buttons in the group
on top of the group box.
4) Contol Properties. With check boxes and control buttons you should
set a linked cell location wherre either TRUE or False will appear in one of
the cells in the worksheet. You will use these cells to tabulate the results
of your questions.

When you first place the items on the worksheet the properties arre
changable like the Caption. The caption should be the selection or answer to
each of your questions.

Another usuful item is the button to enter and exit design mode. To move or
modify the controls you need to enter Design Mode and you need to exit design
mode when you are ready to use the controls. To get to design mode you need
to view another toolbar

View - Toolbars - Control Toolbox

The Triangle is the icon to enter and exit design mode.
