Creating new template - that allows fill-ins - Word 2003

[Millet nursing]

I need to create a template that will allow the user to fill-in information
without moving the rest of the info on the page.

My employer uses Word 2003, and I've tried opening a new document as a
template, as well as saving as a word template file (.dot). Neither option
has allowed the information to "stay-put".

Can anyone help?
