Creating Table Of Contents

After typing 1-1/2 pages of outlines, it dawned on me that having a "table of
contents" with hyperlinks (viewing) and page numbers (printing) would be a
good idea.

My outline is four levels deep, but I only need three levels for TOC.
A.
B.
a.
i.
ii.

How do I create TOC having already started the outline?
