cross-functional process maps (template) & logos in header/footer

I use the cross-functional process map template quite
frequently and would like to place a logo on the top right
portion of the map but the logo keeps moving when I reopen
the file. This is the same for other objects as well, when
I try to place a text box or other shape on the top row of
the template or on the thin narrower row beneath it, the
objects move upon reopen.

Also, does anyone know how to place a picture in the
header or footer? Again, a logo?
