cross-referenced footnotes won't update

Operating System: Mac OS X 10.4 (Tiger)
Processor: Power PC

Help, I'm trading a doc. back and forth with editors and my footnote numbering does not adjust when I add or delete a footnote. I've cross referenced all of them, a couple times now.
