Cust. Complaint Database

I have some knowlege, but how can one create a customer complaint form with
drop-down selections of companies being complained about and more
importantly: How to add a notation field to fill-in as customers call (need
to be quite large) Is there a sample someone can direct me to so I can
modify?

You know: time/date call came in, name, action taken, company,
cust./associate comments, date closed etc...
