Custom appointment form NEED ADVISE

I have a public calendar for our Business Unit. They would like to have a
appointment form that has client fields on it, those fields can not be on a
separate tab, must be on the first tab. That form only should come up when an
appointment is made in the public calendar and not in individual user
calendar.
What would l do to accomplish that?

You would need Outlook 2007 and a COM add-in, installed on every user
machine, to implements a form region that duplicates the first page of the
appointment form and adds your custom fields.
