Custom Dialogue Form Controls Create Error On Page2 Of Report

I have a report that pulls information from a query that receives 3 parameter
inputs from a custom dialogue form. The 3 parameter inputs are date values
that I want to make as the column headings (page headings) on my report. I
successfully added text boxes to the report page header that reference the 3
parameter controls on the custom dialogue form. However, they are only
working on page 1. When I scroll through the pages of the report, from page
2 on "#Name?" is showing rather than the value of the control on the custom
dialogue form. It is only working correctly for page one......Any thoughts.

I have a macro attached to the custom dialogue form that when the user
clicks OK opens the report and closes the form. Again, page 1 of the report
looks good but when I scroll through the rest of the pages the "#Name?" value
appears. Any help would be much appreciated, thanks.
