Custom Form can’t be opened

I created a custom form and saved it in the Personal Forms library.

When I attempt to open an existing item which was created using the custom
form an error message occurs indicating the form cannot be opened and an
ordinary Outlook form will be used instead – of course this does not then
show my custom fields.

Also, when I now attempt to create a new item using the custom form (Tools
Menu, Forms, Choose Form) an error message also occurs indicating the form
cannot be displayed.

Any ideas on how to get Outlook to recognise the form again?

I am using Outlook XP.

Try clearing the forms. Choose Tools | Options | Other | Advanced Options | Custom Forms, then click Manage Forms, then Clear Cache.
