Custom form problems

I'm trying to customize the contacts form in OL2003 using some tutorials
I've found but can't even get started.

When I open a form to customize, 'Design this form' is greyed out.

When I choose a form to customize, when I've finished, 'Publish this form'
is
greyed out.

What am I doing wrong?

I just closed BCM down and restarted Outlook but I still can't use 'Design
this form' or 'Publish form'.
Any other ideas?

New profile, same problem.

Just to make sure I'm doing this right, in the new profile I click new
appointment. A blank appointment form opens. At this point should I be able
to select Design this form from the Tools>Forms menu?

Because I can't, I go to Design a form, choose Contacts and add a text box.
At this point should I be able to select Publish this form from the menu?

That didn't work either. I've tried another machine though, same system and
updates and on that machine I'm getting all the options even with BCM
enabled.

The only real differences I can think of is that where I can customize, it's
on a PC where Office 2003 hasn't been updated to SP1 and where I can't
customize is on a laptop where Office 2003 has been updated.

I guess I'm at the end of the road so it's plan B, ie, does Outlook 2003
allow subcategories in any form?

You can create your own keywords-type field, but there's nothing to link it
to the Categories property.

That's quite baffling about the inability to customize. I'm running 2003 Sp1
and have no problems in that area.

While I'm rifling through Google groups and finding that my problem is not
all that uncommon, I thought I'd just add that on the tools menu, Check
Names and Address Book are also greyed. Does that ring any bells with
anyone?

I know I'm clutching at straws but I've got to get this to work somehow.
