custom form

I would like to create an "abscence form" for our employees to request time
off. I'd like the employee to be able to email this form to a manager - it
get approved and returned to the employee and added to the public calendar
that we use for scheduling appointments? i see that there are forms and that
you can create your own but i'm not sure of how to get started.
