custom forms hangs up

I made and published a custom form for viewing contacts (published in
personal forms library). I installed it using "formswap" so that it is used
as the default form for viewing my contacts. About once a week , I'll try to
open a contact and outlook will say it can't open the form and reverts back
to the default contact form. I then have to open up my saved form and
publish it again, replacing the other one in personal forms library. Then
Outlook will install it and it works okay for a week or so. Is there
something I can do to fix this problem?
