Custom Forms in Contacts Outlook 2000 vs 2003 and Mail merge

We have recently upgraded to Outlook 2003. In our previous version - Outlook
2000, we used custom forms in Contacts. We used these to perform mail merges
with no problem at all.

In Outlook 2003, when we try to run a mail merge the user generated fields
do not appear in the mail merge options, only the Microsoft generated one.

Can anybody help?

Start the merge from Outlook, in the contacts folder, using the Tools | Mail Merge command. Also note that the fields must be defined at the folder level. Use the folder's Field Chooser to add them if necessary.

Sorry but this doesn't work. We have tried putting new fields in and they
appear on the mail merge field options, but not the ones we created in
Outlook 2000. Any other ideas?

As I said earlier, the fields must be visible in the User-defined Fields in This Folder list in the folder's Field Chooser. Add them if they're not there already.
