custom forms in ol2003

i am creating a custom form ipm.note in outlook2003...the send and
receive templates are fine, but when i reply it looses the customer
fields i had created (text boxes, labels etc.)

how do i get the reply mail to include these?

Look on the (Actions) page of the custom form. Under the Reply action, you'll see the name of the form used for replies. Unless you changed it, it will be the default message form (IPM.Note). If you want the reply to use a custom form, you need to set the action to that form's message class. The reply form must also be published either to the Organizational Forms library if it's for internal use in an Exchange environment or to every potential user's Personal Forms library.
