Custom Forms

I am working on a custom message form to act as a phone
message. I customized the message form and added check
boxes and when I send the form to the recipient, the check
boxes have been removed from the message. Does anyone know
why and how to prevent this?

Also, does anyone know who to change the icon in the inbox
for a certain form? You know when you get an email message
in the inbox there is an envelope icon. For my phone
message form I would like to have a phone icon. Is this
possible?

Lastly, I want to add a link in the expanded new button to
have the phone message form. Is this possible? In other
words when I click the arrow on the new button in the inbox
and there are the choices of new message, new contact, new
post etc., I would like to have a new message there. Can I
do that?

Did you create a read layout in addition to the compose layout?

You can change the icon on the (Properties) page of the form.

Thanks for the read layout tip. Fixed that problem.

After I finalized the form and opened it, I went to the
properties page of the form and did not see an option to
change the icon. Please advise.

Look again, on the right side of that page. Options for large and small
icons.
