Custom Icon for custom form not working

Hi,
For my custom task form, the custom icons are not showing properly. I
initially had two custom icons. I assigned thse using the the Property page.
they show up correctly in the property page. Send form definition with this
item is not checked. But the "envelope" icon shows in the task folder view.
In fact when i changed the icons to standard outlook icons in the forms
folder (Notepad with swoosh), the task folder view shows the envelope icon
but the task detail view shows the large icon correctly.
any ideas
