Custom keyboard shortcuts stop working

I've noticed this a few times now in our environment. Two cases that are
similiar:

1. User has a custom keyboard command for inserting a symbol.
2. Another user has a keyboard shortcut to run a macro.

In both cases their keyboard shortcuts do not stick. They can use it for a
day as long as they do not close Word. When they do, and then open Word back
up the keyboard shortcuts no longer work. They still show as assigned, but
they do not activate the symbol or macro. Any ideas?

By default they would be stored in the normal template. Assuming that you
are not doing something stupid like sharing that template between users, it
could be an indication that all is not well with the normal template. See
