custom message form-sends as a blank

I have designed a custom message form, but am having problems when sending
it. The form has various comboboxes, checkboxes, and a message field for
inserting attachments. When I complete the form (including an attachment)
and send it to myself to check the form, all I get is the message box with
the attachment.
I am completely new to designing forms and would appreciate any ideas!

Did you create a separate read layout for the form? Message forms, by default, have separate compose and read layouts.

Umm...No. I wasn't aware of that. How do I go about doing that?

Make sure each control is bound to an appropriate OUtlook property -- use the VAlue tab on the control's Properties dialog.
