custom phone number fields

I found a KB article # 266267 about being about to format custom phone
fields with Intellisense.
I have no VB script experience and the article explains how to use a script
in order to get the field to format like a phone number..

I have several custom phone fields that need to be formatted this way on a
custom form... and I need some guidance on how to accomplish this...

Any help would be appreciated...
