Custom Toolbar and Add-In

Word 2003 XP. Work environment is networked.
(I posted to Word General last week, and no answers yet, I found this group
and am hoping this is where I'm going to likely get pointed to a solution, so
I'm sorry for posting two times).

I have a toolbar created via MSAccess that distributed as a Word Add in.
Only on my machine does this issue exist:

Toolbar works fine until I start a new document based on a template. Then
the toolbar reverts to its previous state (it has been modified with
additions and they show up and work fine until this situation, which is when
it reverts to what it contained before modification).

Again, it is only my computer this happens to. I've replaced my normal.dot.
I've removed .tmp files. I've defragged computer. I do not have Adobe 7,
so I don't have the toolbar that seems to interfere with templates. I have
Adobe 8 Professional.
I've done regedit. We have system-side malware/virus protection. I do not
know what else to do to correct this issue. I created the templates and the
toolbars. My IT team knows less about add ins than I do, so it's up to me to
figure this out. Help would be so appreciated.

I found the solution in another forum: it was an old com.addin that was
causing the problem. The solution for my problem was this:
Word/Tools/Customize/Toolbars - choose Com.addin and put it on the toolbar.
Run the com and it will bring up addins that are running. Remove any addins
that you don't think should be there anymore.
