customize a view in code?

Is it possible to add columns to a view programmatically? I seem to
remember seeing it mentioned somewhere, but can't pin it down.

Yes, it can be done in Outlook 2002 and 2003 with the View.XML property.
Examining that property for a few views will show you the basic layout.
