Hi Steven,
First of all, if by customized toolbar, you mean that you customized the
"Standard" toolbar, then you may be in trouble. Here's why.
Your problem is most likely a corrupt Normal template. The cure is to quit
Word, rename your Normal template (to something like OldNormal), and reopen
Word to force it to create a new Normal. Then you can copy your
customizations over from the OldNormal file to the new Normal using
Organizer, before trashing OldNormal.
The trouble is that the default toolbars, with all your customizations,
can't be copied over. You will have to recreate them. The way to avoid
this the next time (Normal *will* corrupt again eventually), is to create
your own Custom Toolbars in the first place, instead of customizing the
default toolbars. Custom Toolbars *can* be copied from one document or
template to another.
What I suggest is this: While you can still see your toolbars, make a note
of how they're laid out so you can duplicate them. Then test your Normal
template as described above. (If it turns out that it isn't the Normal
template that's at fault, you can rename OldNormal back to Normal and
replace the new one that Word created.)
Once you've got a "clean" Normal, go to Tools> Customize> Toolbars> New and
start from scratch with your own toolbar that won't be jeopardized in the
future.
If you'd stated your OS and Office/Word version numbers (please *always* do
in the future), I could have pointed you to the default location for your
Normal template. If you can't find it on your own, see here:

learn how to use Organizer to restore customizations, see here:

Hope this helps.
