Data Entry Form on intranet web page

Trying to establish and interactive form used on intranet
webpage to have personnel submit supplies requests through
form on intranet and accumulate input to issues supplies
orders. Data Access pages use queries and tables for
basis, but no forms. How can form be used for data entry?
