Database driven form with dynamic Content

Hello All

I am trying to make a form that will change questions based on a
previous answer

Example

Drop down field has dairy
Answers should have milk, cheese, butter...
Drop down has seafood
Answers should have Salmon, Squid, Shrimp...

I need this to be able to talk to a SQL back end for the data and
submit.

Please help if this is possible.
