database for resource distribution

I am new at designing a database. I want to keep track of publications that I
supply to our region on a yearly basis. Does anyone know of a template that
is already designed for this. Just basic information is needed. Name, address
phone, number, what and when publication was sent and how many is basically
all I need. Any suggestions?
