Database Interface Wizard Login to Editor Page Fails to allow logi

I am using FP 2002. I have a problem with the Database Interface Wizard and
the login page for the database editor. When I use the database interface
wizard to create the Database Editor pages and then publish, the login page
does not function. An example can be found by following this page.

For this example, the login username and password are "Admin".
If I use the wizard without creating a password protected login, the pages
created appear to work properly.
Any help with this problem would be greatly appreciated.
