Database Query problem when using search form field

I'm using Frontpage 2002, and I'm using database results wizard to do query
on Access database. When I do the query my criteria set with field equals a
static text value everything works fine. However, when I change the criteria
to a field equal to form field I get the following error with debug on:

Database Results Error
Your page contains a query with user input parameters that could not be
resolved.
This could happen if your DatabaseRegionStart webbot has an empty or missing
s-columnnames or s-columntypes attributes.
You may need to read Microsoft Knowledge Base Article 817029.

I'm not using Frontpage 2003 so asp.net is not an option. I checked for
missing s-columnnames, but all columns were present. Does anyone have any
ideas? Page can be viewed at http://www.engraveditems.com/choose.asp

The 2nd part tells you how to fix it w/o ASP.net
