Datafield to populate check boxes

I have an form that where the data connection is to an Access database. I
have a field for "Missing Documents" that I would like to create Check Boxes
for. How can I create those options that will bind to the "Missing Documents"
field.

I changed it to a check box and I am only getting 1 box for "missing
Documents". I want to be able to list out types of Missing documents as
option boxes. Any suggestions on how can do this?
