Date Change

Hi,

I wish to add a function to allow a date input field [review date],
which can be any day of the week, to be modified so that it displays
as the Monday of the week in which the [review date] falls.

e.g. if [review date] is 17-Jun (Thursday) then [review date] will
display as 14-Jun (Monday).

Can someone help please
