Date Field in a text string

How do I use a date field in a text string? This one is not working?
=" MOPS Change Control Summary - Effective Date: " & " Requested
Completion Date"
Requested Completion Date is my text field.

No Quotes and you need brackets because the field name has spaces in it.

="MOPS Change Control Summary - Effective Date: " & [Requested Completion
Date]
