DB to include questionnaire

Need to create a db to track warranty-type information - which is clear...BUT
there are 10 questions for each warranty - some are Y/N, other questions
require answers - how do I design the TBL?

Would a Q1, Q2 etc., be appropriate? Then the Caption would include the full
length of the question?

Direction from the EXPERTS would help.
