One thing you can do is add a specific file to the start menu icon that launches OneNote, which will cause it to always start up there. To do this:
1) Start->My Computer
2) Local Disk (C
3) click on the "show me these files" link
4) open Program Files
5) click on the "show me these files" link
6) open Microsoft Office
7) open Office 11
8) right-click on Microsoft Office OneNote 2003
9) click "create shortcut"
10) right click on the shortcut and select "properties"
11) in the "target" field, add a file name after the stuff that's already there
a. make sure the file name is in quotes ("c:\documents and settings\owenb\my documents\my notebook\work\misc.one", for example)
b. make sure there's a space between the stuff that's already there and the file name
c. include the complete path to the file (as above)
12) (optional) right click on the shortcut and select "rename" (if you want to change the name from "Shortcut to OneNote" to something like "OneNote <name-of-file>"
13) click on the shortcut and drag it onto the start button (then put it wherever you want it to go in the start menu)
then clicking on this shortcut will open OneNote to the section you've specified every time.

What do you mean by "folder"? OneNote has a default directory (initially
c:\documents and settings\<you>\My Documents\My Notebook and changeable in
Tools|Options|Open and Save) that contains the section/page hierarchy.
Did you mean "page"? OneNote always starts up with the last page that you
accessed displayed. You can keep a shortcut to your desired *.ONE file
somewhere (Startup folder, QL Toolbar, etc.) if you always want that
particular page to open.
How do you determine which folder opens by default? I have created a
new folder which I want as the default when I start OneNote, but I
have to navigate to it each time.
