Default forms being used instead of custom form

I have been using a custom contact form for the past
couple of years. Today whenever I opened an item from the
folder, the default contact form is used. I have checked
to make sure that the custom form is the only one listed
to be used. If I make a new entry, the custom form is used
and can be reopened using the custom form. Any suggestions?
