definitive answer? exchange 2003 settings for entourage X 10.1.4??

Can anyone tell me what settings I need to enter in the account
settings screen when setting up an exchange account?

For the exchange server do I enter the IP address or netbios name? Or
is it the URL?
Do I use http or https?

What's the domain?
All windows users and entourage 2004 users seem to use domain-name.com

our usernames are all firstnamelastname but our e-mail addresses are
firstname@

I'm sure I've got all the settings correct but it always returns...

An error occurred on the IMAP server

We are using small business server 2003 and entourage 2004 users can
connect fine.
I can connect to OWA fine using Safari (not IE as I get "identity
certificate is invalid")

It's just not clear what I need to enter? IP addresses?
http://mail.domain-name.com/exchange? http://192.168.1.3/exchange?
....com/exchange/username??

I'm at a total loss - all the answers I've found seem to contradict
each other.

I assume webdav, IMAP and LDAP are all enabled as entourage 2004 works
fine.

I've tried all possible combinations of usernames, aliases, urls, ip
addresses, domain names and it's driving me nuts.

Might just set up Panther's Mail.app for temporary access.
