An email form file -- an .oft file, right? -- is just a file. Delete it the way you would any other, by selecting it in Windows Explorer and pressing Delete.

Your answer was right on the mark. For a moment I thought I might run into
hidden files. I also found it helpful that when choosing Forms it displays
the address at the top of the window. MPV....Most valuable personage.
