Deleting custom form field group

I had installed, and soon after uninstalled, the ClearContext add-in years ago. It's left behind a custom field group called "ClearContext" that showsup in the field chooser, after all the default groups like "Frequently used fields", etc. It doesn't contain any custom fields, it's just an empty header and it only shows up only in Inbox and Outbox as far as I can tell, and I can't figure out how to get rid of it. Where is this damn thing stored?
