Desparate! Question about Reports

I have designed a report based on entering parameters to pull
information. This works great, but I need the criteria that I enter
to print on the report. I also need to create a prompt to enter a
name that does not need to be saved, just printed on the report. Does
anyone have any ideas? I'm using Access 2000 on Windows XP and have
to have this completed this afternoon.
