Detecting when Appointment Occurs

Is there an event in Outlook that is triggered exactly when an appointment
occurs or is the only event available a reminder event? I would like to
detect appoinments even if they do not have reminders set. I could query for
appointments periodically; but, that can cause issues on slower exchange
servers. Any thoughts?
