Digital Signature...noob question

OK. I'm sure that this is a VERY simple question, and that I have somehow
lost sight of the simple things, but how do I instert a digital signature
block into my form?!?!?!
I need a form that I can route to a supervisor, have him sign off, then
press on to have it approved by my boss's boss.
Optimally, they would each sign off on every row of my table (approving each
item). However, I would settle for one big signature at the bottom of the
form.
Unfortunately, I've searched online, ms forums, ms help, and google. It
seems that everyone but me knows how to insert the blank space for people to
sign!!!

Kim, there is no need to apologize for being new at using digital signatures
