Directory Data Merge

I have created a main document, entered all the data, and when the
merge takes place, some records are missing. When I double check in
the data source, I see the record is there for those that are missing.
In other cases, I see some records are duplicated. The duplication is
not a problem, but the missing data in the merged document is. Any
suggestions?
