'Disappearing' document comments

I've been editing a document and adding comments as I go. For some
reason, some comments will become "invisible" in the reviewing pane.
I'll see [mm8], [mm9] and then a jump to [mm12] for example. The text
for comments mm10 and mm11 will be included with mm9 in the reviewing
pane.

Mousing over the comments in the document shows popups with the
expected text.

If I try to insert a new comment after [mm9], it is not possible. No
[mm9] will be displayed in the reviewing pane.

Kind of aggravating!


I am using Word 2003, build 11.6113.5703.
