Disappearing toolbar button

I have created a template with a custom toolbar button. If I open the
template, the toolbar button is plainly visible. However, if I try to create
a new document based on the template, the toolbar button is gone.

The template is itself based on another custom template, which I suspect
might have something to do with it.

Any ideas what I should do to get my toolbar button back?
