Check with your local Corp Comission, they usually have the basic forms
available online - AZ does; just go to the commission's site. If you're
setting up a simple LLC...it's a piece o'cake, you can do it yourself for
under a 100$. If you're gonna have members other than a spouse it gets more
complicated.
