Drop down lists in seperate columns

I want 2 seperate drop down lists on the same page, drop down list 1 will show customer, street address, suburb, city, these will be underneath each other.
Drop down list 2 in column 2 will show charge client, delivery address, suburb, town, again under each other.
I have tried to achieve this but when I enter info into drop list 1 i get the list showing in a non deliminated form and the same info in both drop down lists

attached is a picture of the result

Any help much appreciated
