On page 4 of the wizard:

1. Under Choose Formatting Options For The
Records Returned By The Query, choose
Dropdown List - One Record Per Item.
2. In the Display Values From This Field dropdown box,
choose Username.
3. In the Submit Values From This Field dropdown box,
choose UserID.
