Duplicate E-mails

Recently I have noticed that few e-mails which i sent internally are getting
duplicated. Recipients reported that they receive multiple copies of same
e-mail. This does not happen with every e-mail going out from mailbox
though.Looks like it happens randomly. Header/Message ID for both original
e-mail & duplicate e-mail is same.

Recepients have checked Rules in Outlook & they confirm that they are not
receiving duplicate e-mails because of Rules.

I am on Outlook 2003 & Exchange 2003.

I have tried below troubleshooting steps but no luck

I have disabled all Outlook Add-Ins
Removed Exchange Scan Registry

Any ideas how to stop duplicate e-mails going out from mailbox ?
