Duplicate issue records in Project Server

We use issues in Project Server and find out for each issue have multiple
records, it looks like each time you do an update for an issue, it will
create a new record, even you try to close an issue (the old one still show
active, the new one show closed).

I am confused about these duplicated records, do you know if it is possible
to force one issue only have one record (or at least have another sequence
id field for the duplicated issues)? Or this is the design style for Project
Server which we can not do anything.

Any information on this matter will be greatly appreciated.

If this happens on every WSS site, then the template is set up with the wrong
filter settings for Issues. Usually, the filter you want to use is Current
is equal to Yes.
